For EtG assessments, additional tests, and/or panels beyond ten, 150mg of the sample is advisable. We suggest using a jeweler’s scale for accurate measurement. If scalp hair cannot be collected, a comparable amount of body hair suffices. When referencing head hair, it strictly pertains to scalp hair, whereas body hair includes all other types such as facial and axillary hair.

Process Overview

The laboratory analysis of a drug test encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves introducing the sample into the lab system, ensuring it was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any data not covered by an electronic chain of custody system.

Screening serves as an initial assessment for substances. While it offers a cost-effective way to dismiss drug usage in most samples, a positive result requires confirmation for legal validation. Samples flagged as presumptively positive in Screening necessitate further verification.

Upon receiving a presumptive positive from Screening, additional hair is extracted from the original sample to prepare it for Extraction. In this phase, substances are drawn from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screening outcome employs GC/MS, GC/MS/MS, or LC/MS/MS methodologies. All tentative positive samples undergo washing before confirmation as needed. The full laboratory workflow from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
