A 1.5-inch sample containing around 200 strands of hair (roughly the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the recommended quantity for both screening and confirmation. For tests such as EtG, add-ons, or panels beyond 10, a sample of 150mg is advised. We suggest using a jeweler’s scale to measure the specimen. If scalp hair is unavailable, a comparable volume of body hair may be utilized. Head hair refers solely to scalp hair, whereas body hair includes all other types of hair (facial, axillary, etc.).

Process Overview

There are four critical phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a specimen into a lab’s system. This process involves confirming that the sample was securely sealed and shipped, assigning it a random LAN (Laboratory Accessioning Number), and completing any additional data entry that isn't already provided by an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of abuse. While Screening is an economical way to eliminate drug presence in most samples, a positive screen requires confirmation to be valid in legal contexts. Any samples showing a positive result in Screening do need a follow-up confirmation.

Should a sample indicate a positive result during the Screening phase, additional hair is extracted from the initial sample for Extraction. At this point, drugs are removed from hair at significantly reduced concentrations compared to other methods (e.g urine or oral fluid), which is why hair drug testing is considered the most challenging method to execute.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are cleansed prior to confirmation when necessary. The complete laboratory process from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
