Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair captures a prolonged record of alcohol and drugs by embedding biomarkers within the fibers of the growing hair strand. When obtained near the scalp, hair can offer roughly a 3-month detection timeframe for alcohol and drugs. Collecting hair is straightforward, tampering is challenging, and it is easy to transport.
A 1.5-inch sample encompassing about 200 strands of hair (comparable to the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is optimal for screening and confirmation. For EtG, additional panels, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is not available, an equivalent quantity of body hair can be gathered. In relation to head hair, we refer specifically to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).
Process Overview
The four primary phases in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.
Accessioning comprises the initial handling of a specimen within a laboratory’s system. This step includes ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not included in an electronic chain of custody system.
Screening consists of an initial expedited check for substances of abuse. While Screening is a cost-efficient method to rule out drug usage in most samples, a positive result necessitates confirmation to be valid in a legal setting. Any samples showing presumptive positives during Screening demand a secondary confirmation.
If a sample shows presumptive positivity in the Screening stage, further hair from the original sample is prepared for Extraction. During this stage, drugs are removed from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug analysis is notably challenging.
Confirmation for any positive Screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive undergo washing before Confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accrediting by ISO / IEC 17025 standards.

Note: Although commonly termed "hair follicle tests," the test itself examines the hair strand and not the follicle beneath the scalp.
