What it is, How it works

Hair can record a lengthy history of substance consumption by capturing biomarkers within the strands of growing hair. When taken close to the scalp, hair may offer up to around a 3-month timeframe for detection of alcohol and other substances. Hair is straightforward to collect, relatively challenging to tamper with, and straightforward to transport.

A sample measuring 1.5 inches of roughly 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp will yield 100mg, which is the optimal quantity for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, a 150mg sample is advised. It's preferable to weigh the sample using a jeweler’s scale. If scalp hair cannot be accessed, a comparable quantity of body hair can be utilized. Head hair denotes scalp hair exclusively, whereas body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with initial processing of a sample into the lab’s system. This step ensures the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary additional data entry missing from an electronic chain of custody system.

Screening entails a quick preliminary check for drug abuse. While Screening is cost-effective for dismissing drug use in most samples, a positive screen must be confirmed for court admissibility. Any samples with a presumptive positive in Screening require follow-up confirmation.

Should a sample show a presumptive positive in the Screening phase, additional hair is extracted from the sample for the Extraction phase. Drugs are isolated from the hair at a much lower concentration than in other methods (such as urine or oral fluid), making hair drug screening among the most complex methods to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives are washed prior to confirmation if necessary. The entire lab process from Accessioning to Confirmation is evaluated following both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.
