A sample of 1.5 inches consisting of about 200 hair strands (approximately the diameter of a #2 pencil) near the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-on tests, and/or tests that exceed 10 panels, 150mg of the sample is recommended. It's advisable to measure the sample using a jeweler’s scale. If scalp hair cannot be collected, an equivalent quantity of body hair is acceptable. When mentioning head hair, it pertains only to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four essential phases in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab's system. This process verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry that is not provided by an electronic custody chain system.

Screening includes a preliminary rapid check for substance abuse. Although Screening is a cost-efficient method for ruling out drug use in most samples, a positive screen must be verified to be court-admissible. Samples that are presumptively positive in Screening need a secondary confirmation.

If a sample is presumptively positive during Screening, more hair is extracted from the original specimen for the Extraction phase. Here, drugs are isolated from hair at a considerably lower concentration than in other methods (e.g., urine or oral fluid), making hair drug analysis the most complex methodology to conduct.

The confirmation of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleaned before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
