A sample measuring 1.5 inches, consisting of roughly 200 hairs (similar in thickness to a #2 pencil) closest to the scalp, offers 100mg of hair, which is ideal for both screening and confirmation. When testing for EtG, add-ons, or more than 10 drug panels, a 150mg quantity is advised. We suggest using a jeweler’s scale for precise measurement. If scalp hair is unobtainable, a comparable quantity of body hair is suitable. When referencing head hair, we mean scalp hair exclusively. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

There are four key stages in the lab analysis of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's system, ensuring it's securely sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing extra data entry omitted by an electronic chain of custody system.

Screening includes a preliminary assessment for drug presence. This cost-effective provisional method rules out drugs in most samples; however, a positive result must be substantiated for legal validity. Samples indicated as positive in Screening require a further confirmation.

Should a sample show a presumptive positive during Screening, additional hair is extracted from the initial sample for further Extraction. Here, substances are isolated from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging to execute.

Confirmation of screened positives occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Presumed positive samples undergo cleansing before confirmation, as necessary. The entire laboratory process is examined under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
