What it is, How it works

Hair analysis is gaining recognition as an effective method for identifying drug and alcohol consumption. Hair captures long-term substance histories by embedding biomarkers within the strands as they grow. When sampled near the scalp, hair can reflect drug and alcohol use over a span of up to three months. It's easy to sample, relatively tamper-resistant, and simple to transport.

A sample measuring 1.5 inches, consisting of roughly 200 hairs (similar in thickness to a #2 pencil) closest to the scalp, offers 100mg of hair, which is ideal for both screening and confirmation. When testing for EtG, add-ons, or more than 10 drug panels, a 150mg quantity is advised. We suggest using a jeweler’s scale for precise measurement. If scalp hair is unobtainable, a comparable quantity of body hair is suitable. When referencing head hair, we mean scalp hair exclusively. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

There are four key stages in the lab analysis of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a lab's system, ensuring it's securely sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing extra data entry omitted by an electronic chain of custody system.

Screening includes a preliminary assessment for drug presence. This cost-effective provisional method rules out drugs in most samples; however, a positive result must be substantiated for legal validity. Samples indicated as positive in Screening require a further confirmation.

Should a sample show a presumptive positive during Screening, additional hair is extracted from the initial sample for further Extraction. Here, substances are isolated from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging to execute.

Confirmation of screened positives occurs through GC/MS, GC/MS/MS, or LC/MS/MS. Presumed positive samples undergo cleansing before confirmation, as necessary. The entire laboratory process is examined under both the CAP (College of American Pathologists) Hair criteria and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection timeframe: Hair tests identify drug use up to 90 days, whereas urine tests have a shorter timeframe.
  • Hard to manipulate: Hair tests are highly resistant to manipulation, assuring more reliable outcomes.
  • Shows usage history: These tests can reflect a pattern of drug consumption over time, beyond just recent activity.

Limitations:

  • Lacks recent detection: Drugs appear in hair only after about 5-7 days.
  • Expense: Hair tests tend to be costlier than other drug testing techniques.
  • Result fluctuations: Hair color and individual growth differences can influence drug metabolite levels in hair.

Note: Despite the term "hair follicle tests", the procedure examines hair strands, not the follicles beneath the scalp.
