For EtG, additional tests, and/or assessments beyond 10 panels, a specimen weighing 150mg is advisable. It is suggested to use a jeweler's scale for weighing the sample. Should scalp hair be unavailable, an equal quantity of body hair can be taken. References to head hair solely mean scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary stages in processing a drug test result at the laboratory are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample upon entry into a lab’s data system. This phase ensures the sample was properly sealed and delivered, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing information not covered by an electronic chain of custody protocol.

Screening is a rapid initial assessment aimed at identifying drug use. While Screening is economical for ruling out drug presence in most samples, court-admissible cases necessitate positive findings to be verified. Any samples with presumptive positive results in Screening require additional confirmation.

If the Screening indicates a presumptive positive, additional hair is extracted from the initial specimen for the Extraction phase. Here, drugs are isolated from hair at markedly lower concentrations than in other approaches (e.g., urine or oral fluids), which renders hair drug analysis the most intricate method to conduct.

Confirmation for any positive screening results is executed using GC/MS, GC/MS/MS, or LC/MS/MS methods. Prior to confirmation, all presumptive positive samples undergo washing as necessary. The complete laboratory process, from Accessioning through Confirmation, adheres to both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
