What it is, How it works

When taken near the scalp, hair can offer roughly a 90-day window for detecting alcohol and substances. It's easy to collect, relatively hard to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp provides 100mg, which is the preferred quantity for assessment and validation. For EtG, additional tests, and/or panels exceeding 10, obtaining 150mg of the sample is advised. It's suggested to use a jeweler’s scale for measuring the sample. If scalp hair isn't present, an equivalent body hair sample can be used. When mentioning head hair, it specifically means scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

There are four principal stages in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the primary handling of a sample within a lab’s framework. This step includes confirming the sample was sealed and sent correctly, generating a random LAN (Laboratory Accessioning Number), and completing other data entry tasks absent from an electronic custody chain.

Screening entails an initial rapid evaluation for substance misuse. Although cost-effective for eliminating drug use in most samples, a positive screen necessitates further confirmation for legal acceptance. Presumptive positive samples in Screening demand additional verification.

Should a sample test as presumptively positive in Screening, more hair is extracted from the original sample for Preparation during Extraction. In this phase, substances are sourced from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluids), making hair drug testing the most challenging method to execute.

Confirmation of any screen-positive findings is via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The comprehensive lab procedure from Accessioning to Confirmation is governed by both the CAP (College of American Pathologists) Hair criteria and accreditation to ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
