If scalp hair is not available, an equivalent quantity of body hair may be collected. By head hair, we specifically mean scalp hair. Body hair encompasses all other categories (facial, axillary, etc.).

Process Overview

The key stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial processing phase of a sample within the lab system. This stage checks that the sample was properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any additional information entry not fulfilled by an electronic chain of custody system.

Screening involves a preliminary rapid evaluation for drugs of abuse. Though Screening is a cost-effective solution for eliminating drug usage in the majority of samples, a positive result requires validation for legal admissibility. Samples assumed positive during Screening must undergo a further Confirmatory process.

When a sample is assumed positive during the Screening phase, additional hair is retrieved from the original specimen and prepped for Extraction. During this stage, substances are extracted from hair at a significantly lower concentration than other means (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to execute.

Confirmation of any positive Screening outcome is performed employing GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive specimens are cleansed before Confirmation as necessary. The full lab procedure from Accessioning through Confirmation is examined under the CAP (College of American Pathologists) Hair designation and approved according to ISO/IEC 17025 standards.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.
