However, for recent usage within the last five days, urine remains the most accurate test.

What it is, How it works

Hair analysis has become recognized as an effective method for identifying substance and alcohol consumption. Hair retains a long-term record of alcohol and various substances by capturing biomarkers within the strands as they grow. When collected near the scalp, hair can offer a detection span of up to approximately 3 months for alcohol and other substances. Hair is easy to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for analysis and verification. For EtG, additional testing, and/or panels exceeding 10, a 150mg sample of hair is suggested. We advise using a jeweler’s scale to weigh the sample. In the case of a lack of scalp hair, a comparable quantity of body hair may be obtained. When mentioning head hair, we solely mean scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The principal stages involved in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a sample within a lab’s system. This process includes validating that the sample was sealed and sent securely, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not available in an electronic chain of custody system.

Screening involves a preliminary check for drugs of abuse. Screening is a cost-effective method to exclude drug usage in most specimens, yet a positive screen must be confirmed to be accepted in legal proceedings. Any samples with presumptive positive results in Screening require secondary confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the original sample and prepared for Extraction. This stage involves extracting drugs from hair at a notably lower concentration than other techniques (e.g., urine or oral fluid), which makes hair drug testing the most challenging method to execute.

Confirmation of any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as necessary. The full laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
