Process Overview

The four primary phases in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the introductory processing of a sample into a lab's system. This entails verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry excluded by an electronic chain of custody system.

Screening consists of an initial brief assessment for drugs of abuse. While Screening is an economical approach to exclude drug usage on most samples, a positive result needs confirmation for court eligibility. Samples presumed positive in Screening necessitate further confirmation.

If a sample is presumed positive in Screening, additional hair is extracted from the original specimen for Extraction. Here, drugs are extracted from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), explaining why hair drug testing is the most challenging method to administer.

Confirmation of any presumptive positive outcome in Screening is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples presumed positive are cleaned before confirmation if necessary. The full laboratory process from Accessioning to Confirmation is inspected under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
