What it is, How it works

When hair is taken directly from the scalp, it offers an alcohol and drugs detection period of up to around 3 months. Collection is straightforward, it’s relatively hard to tamper with, and shipping is uncomplicated.

A 1.5-inch segment of around 200 hairs (comparable to a #2 pencil width) nearest to the scalp will yield approximately 100mg, which is optimal for both screening and confirmation purposes. For EtG, additional tests, or tests over 10 panels, a 150mg sample is suggested. Using a jeweler’s scale for weight is advised. If scalp hair can't be obtained, a similar quantity of body hair can be used. "Head hair" specifically refers to scalp hair, whereas "body hair" encompasses other types such as facial or axillary hair.

Process Overview

The four fundamental stages in the lab analysis of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary processing of a sample into the lab’s system. This process checks that the sample was correctly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry missing from an electronic chain of custody system.

Screening provides an initial rapid test for drugs of abuse. Although Screening offers a cost-efficient approach to exclude drug usage in most samples, confirmed evidence is necessary for legal proceedings. Samples showing presumptive positives in Screening require secondary confirmation.

For samples with presumptive positives at the Screening phase, additional hair is taken from the initial sample for Extraction preparation. Here, drugs are removed from hair at much lower concentrations than in other testing forms (e.g., urine or oral fluid), making hair drug screening the most challenging method to implement.

Positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive undergo washing before confirmation as needed. The full laboratory process, from Accessioning to Confirmation, is reviewed under both CAP (College of American Pathologists) Hair accreditation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
