A 1.5-inch segment of roughly 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for testing and validation. For EtG, add-ons, and/or tests that exceed 10 panels, it is advisable to collect 150mg of the sample. It is recommended to use a jeweler’s scale to weigh the sample. If scalp hair is missing, a comparable amount of body hair can be obtained. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four principal procedures in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial handling of a sample within a laboratory’s system. It comprises validation that the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening consists of an initial swift examination for drug abuse. While Screening is a cost-efficient method to eliminate drug usage in most samples, a positive screening result must be verified to be permissible in court. Any samples that are tentatively positive during Screening require secondary confirmation.

If a sample is tentatively positive during the Screening stage, more hair is removed from the original specimen for Preparation for Extraction. In this phase, drugs are removed from hair at lower concentrations than other techniques (such as urine or oral fluid), making hair drug testing the most complex method to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if required. The entire laboratory process, from Accessioning to Confirmation, is reviewed under the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.
