What it is, How it works

Hair provides an extended record of alcohol and other drug usage by capturing biomarkers within the hair strand's fibers as they grow. When collected near the scalp, hair can reveal up to a 90-day detection window for alcohol and other substances. Hair collection is straightforward, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch sample consisting of roughly 200 hair strands (around the diameter of a #2 pencil) proximal to the scalp yields 100mg of hair, the preferred sample size for testing and validation. For EtG, add-ons, and/or tests exceeding 10 panels, a quantity of 150mg is advised. We suggest using a jeweler’s scale for specimen measurement. If scalp hair is not accessible, an equivalent measure of body hair can be used. By head hair, we mean scalp hair only. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Procedure Overview

The four key stages in the lab process of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a lab’s framework. This includes verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening encompasses an initial rapid assessment for drug abuse. Screening provides a cost-effective method to exclude drug usage in most samples, yet a positive result necessitates confirmation for legal admissibility. Samples that are presumptively positive during Screening require an additional confirmation phase.

If a sample tests positive during the Screening phase, additional hair is extracted from the initial specimen and readied for Extraction. Here, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), explaining why hair drug screening is the most challenging method to execute.

Confirmation of any positive screening output is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is scrutinized under CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 standards accreditation.
