Hair testing is regarded as an effective means for identifying drug and alcohol consumption. It offers a prolonged history of alcohol and drug use by capturing biomarkers within the hair fiber matrix. When extracted near the scalp, hair can provide an approximate 3-month period of detection for alcohol and drugs. Hair collection is straightforward, relatively tamper-proof, and convenient for shipping.
A 1.5-inch hair segment sampled from around 200 strands (similar to a #2 pencil thickness) nearest to the scalp will yield 100mg of hair, the optimal portion for preliminary and confirmatory testing. For EtG, additional analytes, and/or tests exceeding 10 panels, a 150mg sample is advised. Using a jeweler’s scale for weighing the sample is recommended. If scalp hair is inaccessible, an equivalent quantity of body hair is acceptable. Here, head hair refers exclusively to scalp hair, whereas body hair includes all other types (such as facial, axillary, etc.).
Process Overview
The core stages in lab processing for a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the preliminary handling of a sample within a lab's workflow. This phase ensures the sample's seal and shipment were intact, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional manual data input not captured via an e-chain of custody system.
Screening is an initial expedited test for detecting drug misuse. Although an economical technique to exclude drug usage for most samples, a positive screen mandates verification for court legitimacy. All samples showing a preliminary positive in Screening necessitate further confirmation.
If the Screening phase results in a presumptive positive, additional hair is extracted from the initial sample for the Extraction process. During this stage, drugs are isolated from hair at a significantly lower concentration than other methods (e.g., urine or saliva), making hair drug testing the most complex technique.
Confirmation of any screening positives is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary pre-confirmation cleansing. The entire lab process from Accessioning through Confirmation is assessed under both CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.
