A sample of 1.5 inches, containing roughly 200 hair strands (comparable in size to a #2 pencil) near the scalp provides 100mg, which is the optimal sample size for screening and confirmation. For EtG, extended panels, or tests exceeding 10 panels, a 150mg sample is advised. It's suggested to use a jeweler's scale for weighing specimens. If scalp hair isn't available, an equivalent amount of body hair can be used. When talking about head hair, we exclusively mean scalp hair. Body hair refers to all other types, such as facial or axillary hair.

Process Overview

The four fundamental steps in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial introduction of a specimen into the lab’s systems. This process includes ensuring the specimen was sealed and shipped correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input not supplied by an electronic chain of custody.

Screening involves an initial rapid test for drug presence. While it’s a cost-efficient method for ruling out drug use in most samples, any positive result during Screening must be confirmed for legal purposes. All samples with a presumptive positive result in Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is taken from the initial sample for Extraction. This phase extracts drugs from hair at concentrations significantly lower than those from other methods (such as urine or oral fluids), which is why hair drug testing is one of the more challenging methodologies to execute.

Positive results in the Screening phase undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptively positive samples are washed as needed before confirmation. The complete process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accredited to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
