What it is, How it works

Hair testing is now recognized as an effective method to reveal drug and alcohol consumption. Hair captures a long-term record of alcohol and other drugs by trapping biomarkers within the hair strand fibers as it grows. When obtained near the scalp, hair can disclose a detection period of up to approximately three months for alcohol and other drugs. Collecting hair is straightforward, relatively resistant to tampering, and convenient to transport.

A 1.5-inch sample consisting of about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for both screening and confirmation. For EtG, extensions, and/or tests exceeding 10 panels, 150mg of the sample is advised. Employ a jeweler’s scale to measure the sample accurately. Should scalp hair be unavailable, an equivalent quantity of body hair may be collected. When discussing head hair, it refers solely to scalp hair, while body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This step verifies that the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any supplemental data entry absent from an electronic chain of custody setup.

Screening includes a preliminary check for illicit substances. Although Screening is a cost-efficient method to eliminate drug use in most samples, a positive screen requires confirmation to be valid in legal proceedings. Any samples that test presumptively positive during Screening demand subsequent confirmation.

Should a sample register presumptively positive in the Screening phase, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are drawn from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), rendering hair drug screening the most challenging methodology to execute.

Positive screening results are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation as necessary. The comprehensive laboratory process, from Accessioning to Confirmation, adheres to review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
