What it is, How it works

When sampled near the scalp, hair can reveal usage of alcohol and other substances for a detection period of roughly three months. Hair is straightforward to collect, relatively resistant to tampering, and convenient for transport.

An ideal sample requires a 1.5-inch section of approximately 200 hairs (similar in size to a #2 pencil), yielding about 100mg, which is optimal for both preliminary and confirmatory tests. For EtG, add-ons, and/or tests exceeding 10 panels, a larger quantity of 150mg is advised. Utilizing a jeweler’s scale for measurement is recommended. If scalp hair is unavailable, an equivalent amount of bodily hair may be used. The term "head hair" strictly refers to scalp hair, while "body hair" includes all other varieties (facial, axillary, etc.).

Process Overview

The testing of a drug sample involves four primary processes: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples are initially introduced into the lab system, including verifying proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not handled by an electronic chain of custody system.

Screening provides a preliminary scan for drug presence. Although Screening is a cost-effective method to exclude drug use in most samples, a positive screen must be validated for court acceptability. Any sample testing positive in the Screening phase requires a secondary confirmation.

If the Screening outcome is presumptively positive, additional hair is extracted from the primary sample for further Analysis. In this step, drugs are isolated from hair at much lower concentrations than other approaches (e.g., urine or oral fluid), making hair drug screening particularly complex.

The Confirmation of any positive screening result is undertaken using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are cleaned before confirmation if necessary. The entire process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair categorization and complies with ISO / IEC 17025 accreditation standards.
