What it is, How it works

Hair screening has become recognized as a robust mechanism for the identification of drug and alcohol consumption. Hair captures a prolonged timeline of alcohol and other substances by embedding biomarkers within the fibers of the emerging hair strand. When harvested close to the scalp, hair can offer up to a roughly 3-month observation period for alcohol and other substances. Hair is straightforward to gather, relatively hard to tamper with, and convenient to transport.

A 1.5-inch sample of around 200 hair strands (roughly the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal sample for both screening and confirmation. For EtG, add-ons, and/or assessments exceeding 10 panels, 150mg of the specimen is advised. We propose weighing the specimen utilizing a jeweler’s scale. If scalp hair isn't obtainable, an equivalent quantity of body hair may be gathered. When discussing head hair, we are referring specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps involved in the laboratory evaluation of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample into a lab's system. This includes confirming that the sample was secured and shipped correctly, generating a random LAN (Laboratory Accessioning Number), and fulfilling any supplementary data entry not provided by an electronic chain of custody system.

Screening involves an initial expedient review for drugs of abuse. While Screening is a cost-effective strategy to eliminate drug use in the majority of samples, a positive screen demands confirmation to be permissible in court. Any samples that test presumptively positive in Screening necessitate a secondary confirmation.

If a sample tests presumptively positive in the Screening stage, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than other methods (e.g., urine or oral fluid), thus making hair drug screening the most intricate methodology to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
