What it is, How it works

Hair testing has gained recognition as a robust instrument for detecting drug and alcohol consumption. Hair captures a prolonged history of alcohol and substance use by encapsulating biomarkers within the fibers of the growing hair shaft. When gathered close to the scalp, hair can offer a detection window of up to around 90 days for alcohol and other substances. Hair is straightforward to collect, challenging to tamper with, and easy to transport.

A hair sample of 1.5 inches consisting of approximately 200 strands (roughly the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We suggest measuring the specimen using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair can be collected. References to head hair specifically denote scalp hair only. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four principal stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the initial handling of a sample into the laboratory's infrastructure. This process includes confirming proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent in an electronic chain of custody system.

Screening involves a preliminary analysis for drugs of abuse. While screening is a cost-efficient method to rule out drug usage in the majority of samples, a positive screen must be confirmed to be acceptable in legal proceedings. Any samples that test positive in screening necessitate a secondary confirmation.

If a sample is positively flagged during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All samples that are presumptively positive are washed before confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection window: Hair drug tests can identify drug use for up to 90 days, in contrast to urine tests which offer a shorter detection period.
  • Difficulty to manipulate: It is very challenging to adulterate a hair drug test, providing more reliable results.
  • Offers historical insight: It can demonstrate a pattern of drug use over time, not solely recent consumption.

Limitations:

  • Inability to detect recent use: It requires about 5-7 days for drugs to become detectable in hair.
  • Expense: Hair drug tests are typically more costly than other drug testing methods.
  • Result variability: Factors such as hair color and individual differences in hair growth can influence the concentration of drug metabolites in the hair.

Note: Although commonly termed "hair follicle tests," the analysis is performed on the hair strand, not on the follicle below the scalp.

Why Hair Follicle Testing Beats Urine Testing

In Umpqua, OR, hair follicle tests offer a distinct advantage over urine tests with prolonged detection periods of substance use.
