The utilization of hair testing has emerged as an effective approach for detecting drug and alcohol consumption. Hair captures a comprehensive history of substance use by embedding biomarkers within the strands as they grow. When sourced close to the scalp, hair can provide an approximate 3-month detection window for both alcohol and drugs. Hair samples are easy to gather, challenging to manipulate, and convenient for shipping.
A sample measuring 1.5 inches, consisting of roughly 200 strands of hair (similar to the thickness of a #2 pencil) near the scalp, yields 100mg of hair, which is ideal for both screening and confirmation. For EtG, supplementary tests, and/or analysis beyond 10 panels, a specimen of 150mg is advised. It is recommended to use a jeweler’s scale for weighing the sample. In cases where scalp hair is insufficient, an equivalent quantity of body hair can be utilized. The term "head hair" specifically refers to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).
Process Overview
The four primary stages in the laboratory analysis of a drug test result include Accessioning, Screening, Extraction, and Confirmation.
Accessioning pertains to the initial integration of a sample into a laboratory's system, ensuring the sample is properly sealed and transported. This stage involves assigning a unique LAN (Laboratory Accessioning Number) and completing any requisite data inputs not covered by an electronic chain of custody system.
Screening acts as an initial evaluative process for identifying drugs of abuse. Although Screening is an economical method for excluding drug use in most samples, a positive indication must be validated for court admissibility. Samples that yield a presumptive positive during Screening require further confirmatory analysis.
Should a sample indicate a presumptive positive in the Screening phase, additional hair is extracted from the original sample for further analysis in the Extraction stage. This phase involves isolating drugs from hair at a significantly lower concentration compared to other methods (like urine or oral fluid), which underscores the complexity involved in performing hair drug screening.
Confirmatory testing of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Necessary presumptive positive samples undergo washing before confirmation as applicable. The complete lab process, from Accessioning to Confirmation, undergoes review in compliance with the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Limitations:
Note: Although termed as "hair follicle tests", the analysis is based on the hair strand and not the follicle beneath the scalp.
