What it is, How it works

A 1.5-inch snippet consisting of roughly 200 hair strands (comparable to a #2 pencil) closest to the scalp yields 100mg of hair, which is ideal for analysis and confirmation. For EtG, additional tests, and/or panels exceeding 10 types, a sample of 150mg is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. The term head hair refers specifically to scalp hair, while body hair covers all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial setup of a sample within a lab’s system. This process verifies proper sealing and shipment of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data entry not captured by an electronic custody system.

Screening offers a preliminary review for substances of abuse. While cost-effective for eliminating drug use for most samples, a positive screen must be confirmed for it to hold up legally. Any samples showing initial positive results need further confirmation.

For samples identified as positive during Screening, additional hair is extracted from the original sample and set for Extraction. Here, drugs are retrieved from hair at lower concentrations than those in other methods (like urine or oral fluid), making hair drug screening the most challenging to conduct.

Positive screening outcomes undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumed positive results are cleaned before confirmation as required. The lab process from Accessioning through Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
