What it is, How it works

Hair captures a long-term record of alcohol and drug intake by holding biomarkers within the fibers of the hair as it grows. When taken near the scalp, hair offers up to about a 3-month window for detecting drugs and alcohol. Collecting hair is uncomplicated, relatively hard to tamper with, and straightforward to transport.

A sample measuring 1.5 inches with around 200 hair strands (comparable to a #2 pencil) taken closest to the scalp will yield approximately 100mg of hair, considered the perfect sample size for testing. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg sample is suggested. It's advisable to weigh the sample on a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair might be used. When mentioning head hair, it specifically refers to scalp hair only. Body hair encompasses all other types of hair (e.g., facial, axillary).

Process Overview

The four primary stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample into a lab’s system. This step involves checking that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and filling in any additional information not provided by an electronic chain of custody system.

Screening consists of an initial rapid assessment for substances. While Screening is an economical method to exclude drug usage in most samples, a positive result from Screening needs confirmation to be valid in legal settings. Any samples preliminarily positive in Screening must undergo secondary confirmation.

If a sample receives a presumptive positive during the Screening phase, additional hair is extracted from the original collection and prepped for Extraction. In this part, substances are extracted from hair at a much lesser concentration than in different methodologies (e.g., urine or oral fluid), which makes hair drug screening the most intricate methodology to conduct.

Verification of any positive Screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation if necessary. From Accessioning to Confirmation, the entire lab process is reviewed under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.
