What it is, How it works

When collected near the scalp, hair can offer a detection window of up to roughly three months for both alcohol and various other drugs. The collection process is straightforward, alteration is somewhat challenging, and shipping is convenient.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional tests, or tests beyond 10 panels, it is advisable to have 150mg of the sample. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is not available, a similar amount of body hair can be collected. When mentioning head hair, we specifically mean scalp hair only. Body hair includes all other types (like facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial sample processing within a laboratory’s framework. This process involves ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody.

Screening is a preliminary assessment for drugs of abuse. While it is a cost-effective means to dismiss drug usage in most samples, a positive screening result must be confirmed to be court-admissible. Samples that suggest a positive result in Screening will need a secondary confirmation.

For samples that are presumptively positive during Screening, more hair is extracted from the original sample for Processing. The drugs are removed from the hair, but at a significantly lower concentration than in urine or oral fluid tests, making hair drug testing the most challenging methodology.

Confirmation of any positive screening result is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo a wash prior to confirmation, as necessary. From Accessioning through to Confirmation, the whole process is reviewed under the CAP (College of American Pathologists) Hair designation and is accredited to ISO/IEC 17025 standards.
