A sample measuring 1.5 inches with approximately 200 hairs (similar to the thickness of a #2 pencil) near the scalp can yield around 100mg, optimal for analysis and validation. For EtG, additional tests, or more than 10 panels, 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair may be used. Scalp hair refers only to hair from the head, while body hair includes all other types (such as facial or axillary hair).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves bringing the specimen into the lab's system. This step ensures the sample's proper sealing and shipping, assigns it a random LAN (Laboratory Accessioning Number), and completes any further data not included by electronic chain of custody.

Screening offers an initial assessment for substances of abuse. It is a cost-efficient way to eliminate drug use in most samples; however, any positive result requires confirmation to be valid in a legal context. Samples that show presumptive positives in Screening need additional confirmation.

During the Screening phase, if a sample tests positive, further hair is taken from the original sample for Extraction. Here, substances are extracted at a lower concentration than with other methods (e.g., urine or oral fluid), making hair drug testing the most challenging method.

Confirmation for any positive screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, undergoes review under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
