What it is, How it works

Hair analysis has gained recognition as an effective means for the identification of drug and alcohol consumption. Hair offers an extended historical record of alcohol and drug intake by encasing biomarkers in the strands of growing hair. When taken close to the scalp, hair can offer up to roughly a 3-month detection period for alcohol and drugs. Hair collection is straightforward, somewhat challenging to tamper with, and simple to transport.

A sample of 1.5 inches consisting of approximately 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for testing and validation. For EtG, supplementary tests, and/or tests beyond 10 panels, a 150mg specimen is suggested. We advise using a jeweler’s scale for weighing the specimen. If scalp hair isn't available, a similar quantity of body hair can be collected. By head hair, we mean scalp hair exclusively, while body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The fundamental stages in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample into the lab’s system. This process verifies that the sample was properly sealed and delivered, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not available from an electronic chain of custody system.

Screening serves as an initial rapid assessment for the presence of abuse drugs. Screenings are an affordable approach to exclude drug consumption on most samples; however, a positive screen requires verification to be valid in court. Samples that yield a presumptive positive in Screening necessitate an additional confirmation.

If a sample is deemed presumptively positive during Screening, more hair is extracted from the original specimen and readied for Extraction. This phase involves extracting drugs from hair at significantly lower levels than other techniques (e.g., urine or oral fluid), making hair drug analysis the most challenging technique to implement.

Confirmation of any positive screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
