A sample measuring 1.5 inches consisting of about 200 hair strands (similar in diameter to a #2 pencil) closest to the scalp will yield 100mg of hair, the perfect quantity for testing and validation. For EtG, add-ons, and/or assessments exceeding 10 panels, 150mg of the hair specimen is advisable. It's wise to weigh the hair specimen using a jeweler’s scale. In instances where scalp hair is not available, an equivalent quantity of body hair may be used. When mentioning head hair, we are exclusively referring to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab’s system. This step includes confirming that the sample was appropriately sealed and transported, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not furnished by an electronic chain of custody system.

Screening entails an initial cursory review for substance abuse. While Screening serves as a cost-efficient method to rule out substance use in the majority of samples, a positive screen must be validated to be acceptable in court. Any samples testing presumptively positive in Screening require a further confirmation.

Should a sample be presumptively positive in the Screening step, additional hair is extracted from the initial sample and prepared for Extraction. In this step, substances are isolated from hair at a much lower concentration when compared to other techniques (such as urine or oral fluid), accounting for the complexity of hair drug analysis.

Validation of any positive screen result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo cleansing before confirmation as necessary. The entire lab process ranging from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the endorsement to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
