A sample measuring 1.5-inches with around 200 strands (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal sample size for screening and confirmation. For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the sample is advised. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair isn't available, an identical amount of body hair can be obtained. When referencing head hair, it implies scalp hair strictly. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the sample's initial processing into a lab’s system. This involves ensuring the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening comprises an initial rapid examination for drugs of abuse. Although Screening provides a cost-efficient method to exclude drug usage on most samples, any positive screen needs confirmation for court admissibility. Samples showing presumptive positives in Screening do need a secondary confirmation.

If a sample shows presumptive positivity in the Screening phase, additional hair is taken from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a much lower concentration than other techniques (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

GC/MS, GC/MS/MS, or LC/MS/MS processes confirm any positive screening outcomes. All presumptive positive samples undergo a wash before confirmation as required. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
