What it is, How it works

Hair sample collection is straightforward, relatively resistant to tampering, and convenient for shipping.

A hair sample sized at 1.5 inches and consisting of around 200 strands (equivalent to a #2 pencil's thickness) near the scalp yields 100mg, the preferred amount for thorough testing. For more detailed analyses such as EtG add-ons or examinations exceeding 10 panels, a specimen weighing 150mg is suggested. Specimens should ideally be measured using a jeweler's scale. In the absence of sufficient scalp hair, an equivalent portion of body hair can be used. Notably, "head hair" exclusively references scalp hair, whereas "body hair" encompasses all other hair types (such as facial or axillary).

Process Overview

Processing a drug test result in the lab includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into the lab's database. This step ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes any additional entries omitted by an electronic chain of custody.

Screening conducts an initial scan for drug indicators. While an efficient method to eliminate most non-drug using samples, a positive screen outcome necessitates confirmation for legal validity. Presumptive positive results from Screening must undergo a secondary validation process.

Whenever a sample tests presumptive positive during Screening, further hair is extracted from the original specimen for the Extraction phase. This step extracts drug traces from hair at lower concentrations than other techniques (like urine or oral fluid), marking hair drug screening as the most intricate process.

Confirmation of a positive result from Screening is achieved using GC/MS, GC/MS/MS or LC/MS/MS. Any presumptive positives are cleaned prior to confirmation as required. The complete laboratory workflow from Accessioning through Confirmation is subjected to review under the CAP (College of American Pathologists) Hair designation and conforms to ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
