If scalp hair cannot be obtained, the equivalent amount of body hair can be used as a substitute. The term 'head hair' pertains solely to scalp hair. 'Body hair' covers all other types (such as facial, axillary, etc.).

Process Overview

There are four crucial stages in processing a drug test at the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of inputting the sample into the lab's system. It ensures the sample's packaging and transportation were conducted correctly, assigns a random LAN (Laboratory Accessioning Number), and fills in any data gaps not covered by an electronic tracking system.

Screening performs an initial assessment for drugs. While cost-efficient and effective for most, a positive result in Screening necessitates additional confirmation to hold validity in legal proceedings. Any screenings indicating presumptive positives require further verification.

A presumptive positive result in Screening leads to additional hair being extracted from the original sample for the Extraction phase. Here, drugs are isolated at lower concentrations compared to methods like urine or oral fluid tests, necessitating more sophisticated techniques in hair analysis.

Confirmation of any positive results from Screening is carried out using methods such as GC/MS, GC/MS/MS, or LC/MS/MS. All positive samples undergo washing before confirmation as determined necessary. The entire spectrum from Accessioning to Confirmation is assessed under CAP (College of American Pathologists) standards and compliant with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
