For EtG enhancements and/or screenings exceeding 10 panels, it is advisable to obtain 150mg of the sample. Precision in weighing the sample using a jeweler’s scale is suggested. If head hair is unavailable, an equivalent amount of body hair can be utilized. When referencing head hair, this specifically pertains to scalp hair. Alternatively, body hair encompasses other types (such as facial or axillary hair).

Process Overview

There are four fundamental phases in the laboratory assessment of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary integration of a sample into a laboratory's system, verifying correct sealing and shipment of the sample, allocating a random LAN (Laboratory Accessioning Number), and addressing any supplementary data entry not covered in an electronic chain of custody system.

Screening is an initial assessment aimed at identifying potential drug abuse. Though cost-effective, any positive findings in Screening necessitate confirmation for legal admissibility. Presumptive positives identified during Screening require secondary verification.

If Screening suggests a positive result, a portion of hair from the original sample undergoes further preparation for Extraction. Extraction retrieves drugs from hair at lower concentrations than alternative methods (such as urine or oral fluid), posing unique challenges to hair drug screening.

Confirmation of any positive result from screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples receive pre-confirmation washing as necessary. Each step, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair classification and adheres to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
