A 1.5-inch segment of roughly 200 hair strands (approximately the width of a #2 pencil) closest to the scalp equates to 100mg of hair, the optimal sample size for screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. Use a jeweler's scale to weigh the specimen. If scalp hair isn't available, an equivalent volume of body hair may be gathered. When we mention head hair, we specifically mean scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The key stages in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample into a laboratory's system. This comprises validating that the sample was sealed and dispatched correctly, generating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not provided by an electronic chain of custody system.

Screening involves a preliminary quick assessment for drugs of misuse. While Screening is a budget-friendly way to eliminate drug usage in most samples, a positive screen must be confirmed to be permissible in court. Samples that are tentatively positive in Screening do require supplementary confirmation.

If a sample tests tentatively positive during Screening, more hair is withdrawn from the initial sample and prepared for Extraction. In this stage, drugs are drawn out from hair at a substantially lower concentration than in other methodologies (ex. urine or oral fluid), which is why hair drug screening is among the most challenging methodologies.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All tentatively positive samples are cleansed before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
