A sample of 1.5 inches consisting of around 200 strands of hair (about the thickness of a #2 pencil) near the scalp will yield 100mg, which is the perfect quantity for both screening and confirmation. For EtG, additional tests, and/or assessments beyond 10 panels, 150mg is advisable. It is recommended to gauge the specimen's weight using a jeweler’s scale. If scalp hair isn't obtainable, an equivalent amount of body hair can be gathered. "Head hair" specifically refers to scalp hair, whereas "body hair" includes all other types of hair (such as facial and axillary).

Process Overview

The key steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample within a laboratory's framework. This step involves verifying the correct sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening involves an initial and rapid assessment for drugs of abuse. Screening is a cost-efficient method to exclude drug usage in most samples; however, a positive indication requires confirmation to be legally acceptable. Any samples that yield a presumptive positive during Screening must undergo an additional confirmation.

Should a sample yield a presumptive positive in the Screening phase, more hair is retrieved from the primary specimen and readied for Extraction. This stage involves isolating drugs from hair at significantly lower concentrations than other methods (such as urine or oral fluid), which is why hair drug screening is notably challenging to conduct.

Confirmation of any initial positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples that presumptively test positive are cleansed before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation conforming to ISO / IEC 17025 standards.
