What it is, How it works

Hair testing is now regarded as an effective method for recognizing drug and alcohol consumption. Hair offers a prolonged record of alcohol and drugs by capturing biomarkers within the fibers of the hair as it develops. When collected close to the head, hair can offer up to a three-month period for detecting alcohol and other drugs. Hair is straightforward to collect, not easily tampered with, and simple to transport.

A 1.5-inch segment of approximately 200 strands of hair (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect sample for both screening and confirmation purposes. For EtG, additional tests, and/or analyses exceeding 10 panels, a 150mg specimen is advised. We suggest using a jeweler's scale for weighing the sample. If no scalp hair is available, an equivalent amount of body hair is acceptable. When mentioning head hair, we exclusively refer to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The procedure for processing a drug test result in the laboratory consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab’s workflow. This comprises validating that the sample was securely sealed and shipped correctly, generating a random LAN (Laboratory Accessioning Number), and performing any extra data entry that isn't handled by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drugs of abuse. Although Screening offers a cost-effective strategy to eliminate drug use in most samples, legal cases require a positive screen to be confirmed. Samples that appear positive in Screening need subsequent confirmation.

If a sample tests presumptively positive in the Screening process, additional hair is extracted from the initial sample and set up for Extraction. In this step, drugs are retrieved from hair at lower concentrations than in other techniques (such as urine or oral fluid), which renders hair drug screening as the most intricate method to conduct.

Confirmation of any positive screen result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleansed before confirmation if necessary. The entire process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
