What it is, How it works

When hair is collected near the scalp, it can reveal a detection window for drugs and alcohol extending up to three months. This type of testing is easy to gather, hard to tamper with, and straightforward to transport.

Collecting a 1.5-inch portion of roughly 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg, which is optimal for screening and confirmation. For EtG additions or panels exceeding ten, 150mg of the sample is suggested. We advise measuring the sample using a jeweler’s scale. If scalp hair cannot be obtained, a corresponding amount of body hair can be used. When referencing head hair, it strictly means scalp hair. Other hair types (such as facial and axillary) are categorized as body hair.

Process Overview

The testing process in a laboratory incorporates four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of introducing a sample into a laboratory's system. This stage verifies that the sample is sealed and shipped accurately, assigns a random LAN (Laboratory Accessioning Number), and completes any necessary data entry that's missing from an electronic chain of custody system.

Screening involves a preliminary quick check for abused substances. It's an economical way to eliminate drug usage on most samples, but a positive result necessitates confirmation for legal acceptance. Samples that test presumptively positive in the Screening phase require subsequent confirmation.

When a sample shows a presumptive positive at the Screening stage, additional hair is drawn from the original sample and set for Extraction. In Extraction, drugs are isolated from hair at lower concentrations than other methods such as urine or oral fluid, making hair drug testing particularly complex.

Confirmation of any positive screen result is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing as needed before confirmation. The complete process within the lab from Accessioning to Confirmation is validated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
