"Head hair" denotes scalp hair specifically, while "body hair" includes all other types (face, axillae, etc.).

Process Overview

The laboratory workflow for a drug screening result includes Accessioning, Screening, Extraction, and Confirmation steps.

Accessioning is the initial integration of a specimen into a lab's system, verifying it was correctly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data not available through an electronic custody chain.

Screening acts as a preliminary substance check. Though cost-efficient for excluding drug use in many samples, a positive result in Screening mandates Confirmation to be legally valid. Positive tests in Screening need a follow-up confirmation.

If Screening suggests a positive result, further hair from the sample is prepared for Extraction. Here, drugs are isolated from hair at lower concentrations than other methods like urine or saliva, underlining the complexity of hair analysis.

Positive results in Screening undergo Confirmation by GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positives are washed before Confirmation if necessary. The lab procedures, from Accessioning to Confirmation, comply with CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
