A hair sample of 1.5 inches in length comprising approximately 200 strands (about the size of a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, supplementary analyses, or tests exceeding 10 panels, 150mg of the sample is advisable. Using a jeweler’s scale for weighing the specimen is recommended. Should scalp hair be inaccessible, an equivalent quantity of body hair can be collected. By scalp hair, we mean hair from the head, while body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

A drug test result's laboratory processing comprises four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial handling of a sample within a laboratory’s system, ensuring the sample's seals and shipment are intact, generating a random LAN (Laboratory Accessioning Number), and completing any further data entry absent in an electronic chain of custody system.

Screening serves as a preliminary rapid assessment for potential drug abuse. Although cost-effective in excluding drug use in most samples, a positive screen mandates confirmation to ensure court admissibility. Samples that presumptively result positive in Screening necessitate subsequent confirmation.

For presumed positive samples during the Screening phase, additional hair is extracted from the initial specimen for the Extraction stage. Here, drugs are retrieved from hair at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug screening the most challenging methodology to perform.

Confirmation of any positive screen outcomes is completed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation as needed, all presumptive positive samples undergo washing. The full lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
