What it is, How it works

Hair testing has emerged as a robust mechanism for identifying drug and alcohol consumption. Hair offers an extended overview of substance use by embedding biomarkers within the fibers of the lengthening hair strand. When obtained near the scalp, hair can suggest up to roughly a 3-month interval for detecting alcohol and drugs. Hair is straightforward to gather, somewhat resistant to tampering, and convenient to transport.

A sample of 1.5 inches comprising about 200 hairs (approximately the thickness of a #2 pencil) nearest the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or examinations exceeding 10 panels, 150mg of the sample is advised. We advise using a jeweler's scale to weigh the specimen. If scalp hair is missing, a comparable amount of body hair may be acquired. By head hair, we mean solely scalp hair. Body hair includes all other hair forms (facial, axillary, etc.).

Process Overview

The four crucial phases in the lab processing of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the primary processing of a specimen into a laboratory’s system. This involves ensuring that the specimen was adequately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening entails an initial rapid check for drugs of abuse. While Screening is an economical way to rule out drug use across most specimens, a positive screen must be confirmed to be admissible legally. Any specimens presumed positive in Screening do need secondary confirmation.

If a specimen shows presumptive positivity during the Screening phase, additional strands are drawn from the initial sample and readied for Extraction. In this phase, drugs are retrieved from hair at a significantly lower concentration than in alternative methodologies (ex. urine or oral fluid), which explains why hair drug screening is the most challenging methodology to execute.

Confirmation of any presumed positive Screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are washed before confirmation if necessary. The full laboratory progression from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair distinction and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
