A section of hair measuring 1.5 inches and containing roughly 200 strands (similar to a #2 pencil width) nearest to the scalp yields 100mg, which is the preferred quantity for both initial screening and confirmation. For EtG, extras, and tests exceeding 10 panels, a 150mg sample is advised. It's suggested to use a jeweler’s scale for weighing the sample. If scalp hair cannot be collected, an equivalent volume of body hair is acceptable. When discussing head hair, we mean scalp hair specifically. Body hair includes all other types such as facial and axillary hair.

Process Overview

A drug test laboratory processes a result through four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is first entered into the laboratory system. This step verifies proper sealing and shipping of the sample, assigns a Laboratory Accessioning Number (LAN), and carries out any necessary data input not included in an electronic chain of custody.

Screening is a preliminary test for drug abuse. Although Screening is an economical way to exclude drug use in most samples, any positive result must be confirmed to be legally valid. Samples with presumptive positive results in Screening require an additional confirmation step.

If a sample presents a presumptive positive during Screening, additional hair is extracted from the original sample for further testing. This involves extracting drugs from hair, which tends to have a lower concentration than other test materials (e.g., urine or oral fluids), thus making hair drug testing a complex procedure.

Confirmation of a positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Before confirmation, any presumptive positive samples are cleansed as necessary. The laboratory process from Accessioning through to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair guidelines and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
