What it is, How it works

When taken close to the scalp, hair offers up to an estimated 3-month detection range for alcohol and drugs. Hair is easy to gather, not easily tampered with, and simple to send.

A 1.5-inch sample comprising around 200 hair strands (similar to the thickness of a #2 pencil) from the scalp yields 100mg of hair, the ideal quantity for screening and validation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of hair is advised. It is advised to measure the sample on a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair can be taken. When mentioning head hair, it specifically means scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four essential parts of laboratory drug test processing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the preliminary processing of a sample within a lab's system. It involves confirming the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any additional data input not covered by an electronic chain of custody system.

Screening is a preliminary quick analysis for drugs of abuse. Although Screening is a cost-efficient method to dismiss drug usage for most samples, a positive outcome needs confirmation to stand in court. Presumptively positive Screening samples necessitate an additional confirmation.

If Screening indicates a presumptive positive, more hair is extracted from the original sample for Extraction. Here, drugs are extracted from hair at much lower levels than other methods (e.g., urine or oral fluid), explaining why hair drug screening is a complex methodology.

Positive Screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as necessary. The whole laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair category and the accreditation to ISO/IEC 17025 standards.
