For EtG, additional tests, and/or panels beyond 10, a specimen size of 150mg is advisable. It is suggested to measure the specimen using a jeweler’s scale. In cases where scalp hair is not available, an equivalent portion of body hair can be used. "Head hair" refers specifically to scalp hair only, while "body hair" encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory’s system. This step includes confirming the proper seal and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entries not covered by an electronic chain of custody system.

Screening serves as a preliminary assessment for drug use. Though Screening effectively excludes drug usage across most samples, a positive screen necessitates confirmation to be admissible in court. Samples that preliminarily test positive in Screening require further confirmation.

Upon a presumptive positive result in the Screening phase, additional hair is extracted from the initial specimen for the Extraction process. Here, drugs are isolated from hair at significantly lower concentrations than with other methods (e.g., urine or oral fluid), highlighting why hair drug testing is notably challenging to conduct.

Confirmation of any positive screening is carried out through GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo necessary washing prior to confirmation. The complete laboratory process, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
