A segment measuring 1.5 inches and comprising around 200 hair strands (roughly the diameter of a #2 pencil) nearest to the scalp yields 100mg of hair, which serves as the optimal quantity for both screening and confirmation. However, for tests that include EtG, add-ons, and/or more than 10 panels, a 150mg sample is advised. It is recommended to weigh the specimen using a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair can be used. Head hair specifically refers to scalp hair, whereas body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory’s drug test processing involves the key steps of Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample enters the laboratory system; this step includes checking the seal and appropriate shipment of the sample, providing a random LAN (Laboratory Accessioning Number), and handling any supplemental data not included by a digital chain of custody system.

Screening offers an initial assessment for drug presence. Though cost-effective for broad-scale rulings, a Screening positive must be corroborated for legal acceptance. Samples showing presumptive positives during this stage need subsequent verification.

For a sample indicated as presumptively positive, additional hair is removed from the original sample and readied for Extraction. In this phase, drugs are extracted from hair at lower concentrations compared to other techniques (e.g., urine or oral fluid), signifying the complexity of hair drug screening.

Confirmation of any positive match from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive samples undergo washing prior to confirmation if necessary. The complete process from Accessioning to Confirmation is subjected to evaluation under the CAP (College of American Pathologists) Hair designation, following the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
