What it is, How it works

Hair testing is regarded as an effective method for identifying drug and alcohol consumption. Hair serves as a record of sustained alcohol and drug usage by capturing biomarkers within the fibers of the hair as it grows. When hair is clipped near the scalp, it offers a detection timeframe of up to roughly 90 days for alcohol and other substances. Hair collection is straightforward, relatively resistant to tampering, and convenient to transport.

A sample measuring 1.5 inches, consisting of approximately 200 hair strands (similar in size to a #2 pencil) closest to the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation. For EtG, special tests, and/or tests exceeding 10 panels, it is advised to collect 150mg of the sample. It's advisable to measure the sample using a jeweler’s scale. If scalp hair isn't obtainable, an equivalent quantity of body hair might be used. References to head hair specifically pertain to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

There are four pivotal stages in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step where a sample is processed into the lab’s system. This involves confirming the sample was securely sealed and properly shipped, issuing a random LAN (Laboratory Accessioning Number), and fulfilling any extra data input not covered by an electronic chain of custody system.

Screening is a primary rapid assessment for the presence of drugs. Any positive screening results must be confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing potential positivity are cleaned before confirmation, as necessary. The complete laboratory process from Accessioning to Confirmation is overseen according to the CAP (College of American Pathologists) Hair designation, with adherence to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
