Typically, 1.5 to 2 inches of hair is required from the head or body. Hair tests can be used to detect drugs, alcohol/ETG, or other custom testing options.

Recently, hair follicle drug testing has increasingly become a preferred method for drug screening. Employers, courts, and Substance Abuse professionals are often opting for hair tests over traditional urine tests. Hair follicle drug tests are favored by zero-tolerance employers, courts, and probation programs. The main advantage of hair testing is its extended detection window of up to 90 days, observed collection process, and difficulty to tamper with. Nevertheless, for detecting very recent drug use within the past 5 days, urine tests remain the most accurate.

What it is, How it works

Hair testing is increasingly recognized as an influential method for detecting the use of drugs and alcohol. By capturing biomarkers within the strands of hair as it grows, hair offers an extended record of alcohol and drugs. When gathered near the scalp, hair can reveal a detection window for alcohol and other drugs of up to about three months. Hair is easy to collect, hard to tamper with, and straightforward to ship.

A 1.5-inch sample consisting of nearly 200 hair strands (comparable to the width of a #2 pencil) taken closest to the scalp equates to about 100mg of hair, which is the optimal sample for both screening and confirmation. For tests such as EtG, add-ons, and/or those exceeding 10 panels, 150mg of the sample is suggested. It's advisable to measure the sample on a jeweler’s scale. If scalp hair is not present, an equivalent quantity of body hair can be used. Scalp hair pertains exclusively to head hair, whereas body hair includes all other types (such as facial and axillary).

Process Overview

The laboratory steps in processing a drug test result mainly include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial stage of a specimen's processing into a laboratory system. This entails ensuring the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and filling out any additional data not supplied by an electronic chain of custody system.

Screening is a preliminary rapid examination for drugs of abuse. While Screening offers a cost-efficient method to negate drug usage in most samples, a positive result requires Confirmation to stand up in court. Any samples presumptively positive in Screening necessitate a subsequent confirmation step.

If a sample tests presumptively positive during Screening, additional hair is taken from the initial sample for the Extraction phase. Here, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening one of the most complex methodologies.

Confirmation of any positive Screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are subjected to a wash before Confirmation as required. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Long detection window: Hair drug tests can track drug use for up to 90 days, unlike urine tests, which have a briefer detection window.
  • Difficult to cheat: Cheating on a hair drug test is very challenging, leading to more precise results.
  • Offers historical data: It provides a record of drug use over time, not limited to just recent use.

Limitations:

  • Cannot identify recent use: Drugs take about 5-7 days to become detectable in hair.
  • Expense: Hair drug tests typically cost more than other types of drug testing.
  • Result variations: Factors such as hair color and individual hair growth differences can influence the concentration of drug metabolites within the hair.

Note: Although commonly referred to as "hair follicle tests," the actual analysis focuses on the hair strand, not the follicle beneath the scalp.
