What it is, How it works

By capturing biomarkers within the strands of hair as it grows, hair offers an extended record of alcohol and drugs. When gathered near the scalp, hair can reveal a detection window for alcohol and other drugs of up to about three months. Hair is easy to collect, hard to tamper with, and straightforward to ship.

A 1.5-inch sample consisting of nearly 200 hair strands (comparable to the width of a #2 pencil) taken closest to the scalp equates to about 100mg of hair, which is the optimal sample for both screening and confirmation. For tests such as EtG, add-ons, and/or those exceeding 10 panels, 150mg of the sample is suggested. It's advisable to measure the sample on a jeweler’s scale. If scalp hair is not present, an equivalent quantity of body hair can be used. Scalp hair pertains exclusively to head hair, whereas body hair includes all other types (such as facial and axillary).

Process Overview

The laboratory steps in processing a drug test result mainly include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial stage of a specimen's processing into a laboratory system. This entails ensuring the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and filling out any additional data not supplied by an electronic chain of custody system.

Screening is a preliminary rapid examination for drugs of abuse. While Screening offers a cost-efficient method to negate drug usage in most samples, a positive result requires Confirmation to stand up in court. Any samples presumptively positive in Screening necessitate a subsequent confirmation step.

If a sample tests presumptively positive during Screening, additional hair is taken from the initial sample for the Extraction phase. Here, drugs are extracted from hair at much lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug screening one of the most complex methodologies.

Confirmation of any positive Screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are subjected to a wash before Confirmation as required. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
