For EtG, supplementary tests, or panels exceeding 10, a specimen of 150mg is suggested. It's advisable to measure the sample using a jeweler’s scale. If there's no scalp hair, an equivalent quantity of body hair might be sourced. Note that head hair refers solely to scalp hair, while body hair encompasses all other varieties (such as facial, armpit, etc.).

Process Overview

There are four principal phases in processing a drug test in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the primary steps of integrating a sample into a lab's system. This process checks that the sample is sealed and dispatched correctly, assigns a unique LAN (Laboratory Accessioning Number), and handles any data input missing in an electronic chain of custody system.

Screening offers a basic preliminary check for substance abuse. While it's a cost-effective strategy to eliminate drug use for most samples, any positive outcome necessitates confirmation to ensure courtroom admissibility. Presumptive positives in Screening must go through secondary validation.

For positive screening results, Confirmation is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptive positive samples are often washed as needed. The complete laboratory procedure from Accessioning to Confirmation is carried out with oversight under CAP (College of American Pathologists) Hair criteria and in line with ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
