What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol consumption. Hair records a long-term history of substance use by embedding biomarkers within the fibers of the hair shaft as it grows. When collected near the scalp, hair can offer an estimated 3-month detection period for alcohol and drugs. It's easy to collect, somewhat hard to tamper with, and straightforward to mail.

A 1.5-inch segment of about 200 hair strands, approximately the diameter of a #2 pencil, closest to the scalp yields 100mg of hair, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the hair sample is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair is not obtainable, a similar quantity of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process for drug test results consists of Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary processing of a sample within a lab’s system. It involves ensuring the sample was sealed and shipped correctly, generating a random LAN (Laboratory Accessioning Number), and filling out any additional data entry not captured by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug use in most samples, a positive screen requires confirmation to be valid in legal settings. All samples that are presumptively positive in Screening require further confirmation.

If a sample tests presumptively positive during Screening, more hair is extracted from the initial sample and prepared for Extraction. Drugs are extracted from hair at a much lower concentration than other methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most complex method to execute.

Confirmation of any positive screening outcome is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if necessary. The entire lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair category and compliance with ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug tests can reveal drug consumption for up to 90 days, in contrast to urine tests, which have a shorter detection span.
  • Hard to deceive: Cheating on a hair drug test is extremely challenging, increasing the accuracy of the results.

  • Incapable of detecting recent use: It takes about 5-7 days for drugs to become discernible in hair.
  • Expense: Hair drug tests generally incur higher costs than other drug testing methods.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
