A sample measuring 1.5 inches with approximately 200 strands (resembling a #2 pencil in thickness) from the scalp provides 100mg, perfect for screening and confirmation. For tests involving EtG, additional testing, and/or more than 10 panels, 150mg is preferred. It is advisable to measure the sample with a jeweler’s scale. If scalp hair isn't available, a similar amount of body hair might be used. When discussing head hair, we strictly mean scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The drug test result processing in a lab involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab’s framework. This step checks the sample’s seal and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any extra data input absent from an electronic chain of custody system.

Screening is about a rapid examination for drug presence. While cost-effective for eliminating drug usage in most samples, a positive screen needs validation for court use. Any presumptively positive samples from Screening require subsequent confirmation.

If Screening suggests a positive result, more hair from the initial sample is utilized for Extraction, where drugs are withdrawn from hair at much lower concentrations than methods involving urine or oral fluids, showcasing hair testing as one of the toughest methods.

GC/MS, GC/MS/MS, or LC/MS/MS confirm any positive screening results. Presumptively positive samples undergo a wash before confirmation if needed. The end-to-end laboratory procedure from Accessioning to Confirmation is audited under the CAP (College of American Pathologists) Hair designation and follows ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
