What it is, How it works

Hair analysis is now recognized as an effective method for detecting drug and alcohol use. The hair captures biomarkers within its fibers, recording a prolonged history of alcohol and other drugs. When cut near the scalp, hair can provide an inspection period of up to about 3 months for alcohol and other substances. Hair is easy to gather, somewhat resistant to tampering, and convenient to transport.

A sample of 1.5 inches consisting of roughly 200 hair strands (about the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, which is the perfect sample for both screening and confirmation tests. For EtG enhancements and/or tests exceeding 10 panels, a quantity of 150mg is suggested. We advise using a jeweler's scale to weigh the sample. If scalp hair is not available, an equal volume of body hair can be gathered. In our context, head hair specifically refers to scalp hair only. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory processing for drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab's system, which includes confirming the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and filling out any extra data entry not supplied by an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of abuse. While Screening efficiently discards drug usage for most samples, a positive result needs confirmation to be valid in legal settings. Samples with presumptive positives from Screening need secondary verification.

For any sample testing presumptively positive in the Screening space, further hair from the original sample is prepared for Extraction. This stage involves extracting drugs from the hair at a much lower concentration compared to other methods (ex. urine or oral fluid), making hair drug testing one of the most challenging methodologies to perform.

GC/MS, GC/MS/MS, or LC/MS/MS methods conduct the Confirmation of any positive screening findings. All presumptive positive samples are washed before Confirmation if needed. The entire procedure — from Accessioning to Confirmation — undergoes scrutiny under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
