What it is, How it works

Hair offers a prolonged history of alcohol and drug use by holding onto markers within the fibers of the growing hair strand. When retrieved near the scalp, hair can provide up to a roughly 3-month detection duration for alcohol and drugs. Hair is straightforward to gather, somewhat challenging to tamper with, and easy to transport.

A sample size of 1.5 inches featuring around 200 hair strands (approximately the width of a #2 pencil) closest to the scalp yields 100mg of hair, the preferred sample for analysis and verification. For EtG, additional tests, and/or assessments above 10 panels, 150mg of the sample is advised. It is recommended to measure the specimen using a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair may be collected. Head hair specifically refers to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial handling of a sample within a lab’s system. This process checks that the sample was sealed and sent appropriately, assigns a random LAN (Laboratory Accessioning Number), and completes any further data entry not accounted for by an electronic chain of custody system.

Screening involves a preliminary rapid examination for drugs of abuse. Although Screening is a cost-effective method to rule out drug usage in most samples, a positive result needs confirmation to be valid in court. Any samples suspected to be positive in Screening must have an additional validation.

If a sample shows presumed positivity during the Screening phase, additional hair is drawn from the original sample and readied for Extraction. At this stage, drugs are extracted from hair at a significantly lower concentration than in other techniques (ex. urine or oral fluid), which is why hair drug screening is the most challenging process to execute.

Confirmation of any initial positive screen result is conducted through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleaned before confirmation as needed. The full laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
