For EtG, additional tests, and/or testing beyond 10 panels, 150 mg of the sample is suggested. Weight measurement of the sample on a jeweler’s scale is advised. If scalp hair is absent, an equivalent amount of body hair may be obtained. "Head hair" specifically refers to scalp hair. "Body hair" considers all other types (facial, axillary, etc.).

Process Overview

The procedure within the laboratory for processing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample into the laboratory's system. This process verifies that the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any unanswered data entry not provided by an electronic chain of custody system.

Screening entails an initial rapid evaluation for substance abuse. While Screening efficiently excludes drug usage on most samples, a positive result must be validated to be permissible in court. Any samples that yield a presumed positive in Screening necessitate a secondary verification.

In the event of a presumptive positive in Screening, additional hair from the initial specimen is prepared for Extraction. During this phase, drugs are extracted from hair in a concentration much lower than other methods (such as urine or oral fluid), making hair drug screening the most complex method to execute.

Verification of any positive screening result is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumed positive samples are cleansed before confirmation as needed. The entire lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
